The best thing to use is the underwear made for drysuits. I do admit however to often using just a bunch of different long underwear, t-shirts, long sleeve pullover shirts/light sweaters, sweat pants and a pile of socks. Enough stuff where I can change out almost every day or two at most to a fresh set.
Neoprene suits need less insulation than shell suits and are more forgiving in that regard.
